How should I prepare for my appointment?
Please be sure to bring your insurance card with you to your office visit with Dr. Caskey.
Osteopathic manipulative treatment, FSM, and medical acupuncture are hands-on treatments. Wearing loose clothing or shorts and a T-shirt is advised.
Patients are encouraged to eat a healthy meal and hydrate with water prior to your appointment.
What are your Office Policies?
Should you need to cancel or reschedule an appointment please contact our office as soon as possible, and no later than 48 hours prior to your scheduled appointment. This gives us time to schedule other patients who may be waiting for an appointment.
Any established patient who fails to show or cancels/reschedules an appointment and
has not contacted our office with at least 48 hours notice will be considered a No Show and charged a $75.00 no show fee.
If a third No Show or cancellation/reschedule with no 48 hour notice should occur, the patient may be dismissed from Dr. Justin Caskey’s medical practice.
We reserve the right to not reschedule any new patient who fails to show for their initial visit.
The $75 no show fee is charged to the patient, not the insurance company, and is due at the time of the patient’s next office visit.
We understand there may be times when an unforeseen emergency occurs and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances please contact our office as soon as possible.
What Insurance Policies do you accept?
Dr. Justin Caskey participates with the following major plans:
- Aetna
- Anthem Blue Cross and Blue Shield
- ConnectiCare
About insurance and payments
You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or Dr. Caskey does not participate with your health plan, full payment also is expected at the time of service.
Patients who do not have accepted insurance may obtain an HCFA (Health Care Financing Administration) form to submit to their insurance for possible reimbursement. HCFA forms are available from Dr. Caskey’s office and may be given to you following your visit.
For your convenience, we accept cash, check, MasterCard, VISA, and Discover. All returned checks will be assessed a $35.00 service charge.
